Incidence of fragility in institutionalized patients of an emerging country

Frailty means little resistance or easily broken and has been conceptualized as a state of advanced age and vulnerability characterized mainly by weakness and decrease of the organic reserve before a stressor event. The prevalence of frailty in centers of residence is observed in 15-70% of institutionalized older adults, but in Mexico, there are no data on this. Recognizing the frailty syndrome in the elderly allows it to be prevented and intervened to improve the prognosis, as well as to recognize when palliation is necessary to optimize the costs in the medical care of the hospital system. Observational, descriptive, analytical, retrospective, and cross-sectional study of cases and controls; carried out at the Arturo Mundet National Gerontological Center with 177 patients institutionalized in the period from 2010 to 2015. The incidence of frailty during institutionalization was investigated, and association analysis was performed with the general data, the presence of comorbidities and geriatric syndromes using SPSS v. 20 for the analysis of the same. The majority of the patients were older than 80 years; women and 50% had at least 6 years of schooling. Malnutrition and the risk of malnutrition (60%) were the most prevalent geriatric entity to institutionalization followed by cognitive impairment. Frailty during institutionalization was documented in 72 patients, of whom 37 have already died at the time of observation (p.001278). Age, years of schooling, presence of malnutrition, alterations in the central nervous system, cognitive deterioration, and dementia were associated with the presence of the same. Most geriatric syndromes that involved cognitive function or performance as well as the greater number of geriatric syndromes were associated with the diagnosis of frailty. Frailty requires a multidisciplinary approach, but beyond that, the detection of those patients at risk of presenting it should help us to generate interventions that improve their prognosis
