INTRODUCTION Importance of atherosclerotic carotid artery disease and generalities on preventive interventional strategies Worldwide, a third of all strokes (S) are attributed to arterio-arterial embolism, directly attributable to carotid atherosclerotic disease. In Mexico, the results of a prospective, multicenter, hospital study established that the frequency of major artery disease represents 8.4% of all cerebral ischemic entities. However, it is probable that this figure under represents the real frequency since the rate of ischemic events of indeterminate origin in said registry was high (41%). This may be due to a lack of scrutiny when in search of large vessel atherosclerotic disease particularly in the carotid arteries, hence impacting the possibility of implementing invasive procedures such as angioplasty with stent placement or in particular carotid endarterectomy, the strategy with most impact in secondary stroke prevention.
